Eligibility

Partnership Requirements

To be eligible for Partnership, you must be:

  • Be at least 18 years old with a physical, intellectual, or developmental disability or a frail elder.
  • A resident in one of the counties where Family Care Partnership is offered; click HERE for a list counties where Community Care offers Family Care Partnership.
  • Functionally eligible as determined by the Wisconsin Adult Long-term Care Functional Screen.
  • Financially eligible for Medicaid. (You may also be eligible for Medicare. If you are eligible for Medicare, you must be enrolled in Medicare parts A, B and D and obtain your Medicare coverage from the Community Care Partnership Program. If you are eligible for Medicare, you must have an applicable Special Election Period (SEP) to enroll. If you have questions about SEPs, please contact your ADRC for assistance).
