Certified Adult Family Homes

Certified Adult Family Homes are member specific settings and only necessary for a small percentage of our members. Currently, certified 1-2 Bed Adult Family Homes are not being added to our network unless a specific member need is identified that cannot be served by an existing contracted provider.

If your home is currently certified, you are welcome to apply to join our network; however, your application to contract for a Certified Adult Family Home will be kept on file. In the event Community Care identifies a network need for your home in the future, we will contact you.

If your home is not currently certified, please contact us at contractinquiries@communitycareinc.org. We will request details from you which will be kept on file. In the event Community Care identifies a network need for your home in the future, we will contact you.

Community Care follows the Standards for Certified 1-2 Bed Adult Family Homes (P-00638).
