Our Commitment to Quality

Community Care is dedicated to providing quality care to those we serve. Our member-centered programs strive to improve the quality of life for seniors and adults with physical, intellectual, or developmental disabilities by helping them live as independently as possible for as long as possible within their communities.

Members play an important role in providing feedback to the organization regarding the effectiveness of services and/or suggestions for improvements. If any members, or their legal decision makers, are interested in participating or learning more about the Member Advisory Committee, please contact the Community Care Quality Department.

Annually, a Quality Plan is developed and monitored that includes improvement initiatives and Community Care specific health care data. Please view our Quality Plan to learn about our improvement projects. Here are some of the initiative areas:

  • Monitoring of Annual and Semi-Annual Health Assessments
  • Care Management for Vulnerable High Risk Members
  • Member Satisfaction
  • Member Incidents
  • Appeals
  • Grievances
  • Competitive Integrated Employment
  • Decreasing Staff Turnover
  • Influenza and Pneumonia vaccines

Satisfaction levels, along with other Managed Care Organization rankings, are posted on the Wisconsin Department of Health services website and can be reviewed at: www.dhs.wisconsin.gov  (In the search box enter the program desired.  Example:  Family Care MCO Scorecard, Partnership MCO Scorecard, or PACE MCO Scorecard)
