Partnership Member Handbooks

Welcome to Community Care's Family Care Partnership program (also known as Partnership).

Partnership is a Medicaid program that helps older adults and adults with physical, developmental, or intellectual disabilities. It mixes health and long-term support services in home and community settings.

The Partnership Member Handbook provides information you need to:

  • Become familiar with the medical care and long-term care services in the benefit package.
  • See which services Partnership covers.
  • Know your rights and responsibilities.
  • File a grievance or appeal if you have a problem or concern.
Please click on the link below to download a copy of your Partnership Member Handbook.

If you need help reviewing this handbook, please contact a member of your care team.
