At Community Care, our goal has always been to help individuals with long-term care needs to live as independently as possible for as long as possible in their home or community.
Religious Orders We Serve
For more than 45 years, we have served some of our state’s most vulnerable residents – frail seniors (55 and older) and adults (18 and older) with physical, intellectual, or developmental disabilities. We are also honored to serve three Milwaukee area religious orders: the School Sisters of St. Francis, the School Sisters of Notre Dame, and the Sisters of St. Francis Assisi.

Through the years, we have built a reputation for providing high-quality, cost-effective services and are proud to be the only managed care organization in the state to offer all three major long-term care programs – Family Care, Family Care Partnership, and the Program of All-inclusive Care for the Elderly (PACE).

In addition to caring for our members out in the community, we
also operate two certified adult day centers in Milwaukee and Waukesha. Our day
centers are open to both private pay clients and members enrolled in our programs.

Eligibility for any Community Care program is based on functional and financial need. Age and residency requirements vary by program.
