Tour Community Care to Celebrate National PACE Month

BROOKFIELD, Wis. – September is National PACE Month, and Community Care, Inc. invites the community to see firsthand how the Program of All-Inclusive Care for the Elderly (PACE) empowers seniors to remain independent, connected, and cared for in their own homes. The theme of this year’s observance is #ThankYouPACE to honor the experiences of participants, caregivers and PACE staff and highlight the meaningful, personalized support that defines the PACE model of care.

Community Care offers the PACE program in four Wisconsin counties: Kenosha, Milwaukee, Racine, and Waukesha, with fully licensed Adult Day Centers available in Milwaukee, Racine, and Waukesha. To qualify for the PACE program, individuals 55 and older must apply by contacting their local Aging and Disability Resource Center, which helps people apply for the various available statewide programs and benefits, including long-term care. Once enrolled, individuals can select PACE as their program choice. To qualify for the program, they must be a resident of Kenosha, Milwaukee, Racine or Waukesha County.

PACE is a proven, community-based model of care that provides medical care, social services and daily support for older adults. The program significantly improves the quality of life for participants and their caregivers through services such as prescription drug coverage, meals and transportation.

Care is provided by an interdisciplinary team of professionals. Primary care providers, nurses, social workers, therapists, home care coordinators and transportation staff work together to deliver comprehensive, person-centered care. As a result, 94 percent of PACE participants are able to continue living in their community rather than an institutional setting.

The importance of PACE continues to grow as the country approaches a major demographic milestone. By 2034, older adults will outnumber children for the first time in U.S. history. This shift presents challenges for older adults and their families, from access to affordable housing and caregiver support to workforce shortages, long-term care financing, and the need for technology that enables safety and independence. PACE provides unique solutions to address each of these challenges.

Throughout September, PACE organizations across the country will host events celebrating their communities and the people they serve. As part of the #ThankYouPACE campaign, individuals are invited to share stories and messages of appreciation.
