Community Care and CareSource Affiliation Now Official
Community Care, Inc. (CCI) is proud to announce that its affiliation with CareSource is now official, following all required state and federal approvals.
Originally announced in September 2024, this new partnership brings together two nonprofit, mission-driven organizations with a shared focus: helping people live independently and thrive in their communities.
What This Means for Our Members
For more than 45 years, Community Care has supported older adults and adults with disabilities across Wisconsin. Through this affiliation, we will continue delivering the same high-quality, community-centered care our members rely on - now strengthened by additional resources, innovation, and expanded capabilities from CareSource.
Importantly, there is no disruption to member coverage, services, or care teams. Community Care will maintain its headquarters and employees in Wisconsin.
Strengthening Care Across Wisconsin
Community Care currently serves more than 15,000 members enrolled in Wisconsin’s long-term care programs, including Family Care, Family Care Partnership, and PACE. Our services include:
- Care coordination
- Home and community-based services
- Personal care assistance
- Adult day and health centers in Milwaukee, Racine, and Waukesha
As the need for long-term care continues to grow, this affiliation positions us to better serve Wisconsinites with complex health needs—today and into the future.
