Rights & Responsibilities

As a member of Community Care’s Family Care program, you and your care team will work together to make decisions regarding your health and lifestyle. Together you will make the best possible choices to support you. You are a key participant in your care team and you have rights and responsibilities as a Family Care member. Your team will honor your rights and is dedicated to helping you understand and carry out your responsibilities in order to achieve your goals.
