Family Care Partnership Program The Family Care Partnership Program is a modified version of the PACE model. It is similar to the PACE model except there is less dependence on adult day health centers and the participant has the opportunity to choose a community-based physician from a list of health care providers. Community Care operates Family Care Partnership Programs in Kenosha, Milwaukee, and Racine Counties, and beginning in early 2009 in Ozaukee, Washington and Waukesha Counties. How the Family Care Partnership Program works The Family Care Partnership program features the same components as the PACE model, except that it allows participants to keep their current doctor, if he or she is in our network of contracted physicians, while receiving the benefits of this special program. Individuals enrolling in the Family Care Partnership Program receive all-inclusive health care while continuing to live in the community. Services are delivered in the doctor’s office, in the participant’s home or through an adult day center. The complete linkage of services makes this program convenient and effective. The Interdisciplinary Team will arrange for participant care and will provide transportation to and from medical appointments. Staff may attend appointments with participants to relay important information about any changes in condition to all team members. |
