Family Care Partnership Financing

Community Care receives monthly per capita (or per person) payments from Medicaid, and where applicable, from Medicare, for all eligible enrollees. Family Care Partnership Program members who qualify Medicaid, pay no out-of-pocket costs for services they may have a small copayment for prescription drug medications, and there are no other out-of-pocket costs for services. Once enrolled in the Family Care Partnership Program, you will not be charged for any of your health care services.

Once enrolled, Community Care will arrange and pay for all services recommended by your interdisciplinary team. Your Community Care Team continues your care even if you are hospitalized or in a nursing or group home. If you need additional specialty services, your Community Care team will arrange and pay for them.
