Community Care offers Family Care Partnership Program, as innovative alternatives to nursing home care for eligible participants.

To be eligible for the Family Care Partnership Program, you must:

• Be at least 18 years old (55 years old in Milwaukee County)
• Live in Kenosha, Milwaukee, Ozaukee, Racine, Washington, or Waukesha Counties*
• Meet a level of care as determined by the Wisconsin functional screen
• Be eligible for Medicaid

  * Waupaca County will open July 2010
