Notice for Potential for Contract Termination

Medicaid (the Wisconsin Department of Health Services (DHS)) and Medicare (the Centers for Medicare & Medicaid Services (CMS)) must approve Community Care’s Family Care Partnership Programeach year. You can continue to get coverage as a member of our plan only as long as we choose to continue to offer the plan for the year in question and the DHS and CMS renews its approval of the plan.

If the contracts between Community Care and DHS and CMS are not renewed or terminated, this may result in the termination of your enrollment in the Community Care Family Care Partnership Program (HMO SNP) or in the Community Care Program of All-Inclusive Care for the Elderly (PACE). In addition, Community Care may reduce its service area and has the right to no longer offer services in the area where you reside. If this occurs, you will receive another notice and instructions for continuity of care.

 

 

Financing quick facts

Most participants pay no out-of-pocket costs for services. Once enrolled, other than the possible monthly cost share, you will not be charged for any of your health care services.
