Family Care Financing

Community Care receives monthly per capita (or per person) payments from Medicaid,  for all eligible enrollees. Family Care Program members who qualify Medicaid, pay no out-of-pocket costs for services. Once enrolled in the Family Care Program, you will not be charged for any of your long-term care services.

Once enrolled, Community Care will arrange and pay for all services recommended by your interdisciplinary team. Your Community Care Team continues your care even if you are in a nursing or group home.
