Community Care offers Family Care Program, as innovative alternatives to nursing home care for eligible participants.

To be eligible for the Family Care Program, you must:

• Be at least 18 years old
• Live in Kenosha, Ozaukee, Racine, Sheboygan, Washington, or Waukesha Counties
• Meet a level of care as determined by the Wisconsin functional screen
• Be eligible for Medicaid
