What We Do
Supporting Independence

Our mission today is the same as it was in 1977 when we were founded – to help our members retain their independence and live as independently as possible for as long as possible within their communities. 

Community Care offers three programs in eastern Wisconsin that champion independence – Program of All-Inclusive Care for the Elderly (PACE)Family Care and Family Care Partnership.

Each program differs in specifics, but all three are designed to help our members remain in their home and in their community.

The highlights of our programs include:

  • Making independence possible: We’re dedicated to ensuring our members live in a home setting, as opposed to an institution, whenever possible.
  • Coordinated care approach: A dedicated team of Community Care professionals is assigned to each of our members to help them plan their care needs.
  • Members make decisions: Members and their families work with the care team to determine which types of services and supports that will best meet the member's needs and achieve his/her outcomes.
  • Match members to the services they need: We coordinate services to match the unique needs of each member.
