PACE Eligibility

Community Care offers PACE or Program of All-inclusive Care for the Elderly, as innovative alternatives to nursing home care for eligible participants.
To be eligible for the PACE Program, you must:

Be at least 55 years old
Live in Milwaukee or Waukesha County
Meet a level of care as determined by the Wisconsin functional screen
Be eligible for Medicaid and/or Medicare

Enrollment means security

Once enrolled in Community Care, you remain enrolled for life, regardless of your changing health needs. As a Community Care participant, your needs are met, whether you need hospitalization or nursing home care.
