Community Care’s excellence is recognized
At the forefront nationally in developing innovative, affordable health care and social services for older adults, Community Care has been recognized frequently since its inception in 1977 for its wide-ranging contributions and creative solutions to caring for older adults. Major national grants have been received from the Kellogg Foundation, Robert Wood Johnson Foundation, John A. Harford Foundation and numerous local foundations, including Helen Bader, Faye McBeath and the Greater Milwaukee Foundation. These grants have enabled us to participate in national program initiatives to implement alternative programs for community-based care for the older adult. Our recent recognitions include:
- Milwaukee Awards for Neighborhood Development Innovation (MANDI) has named Community Care - Vliet Street facility as a finalist in the State Farm Building Blocks - large project division that recognizes outstanding real estate development.
This is the 9th year that LISC Milwaukee has held the awards.
- Tau Award bestowed to Community Care by Clement Manor and School Sisters of St. Francis.
Community Care is honored to receive the Tau award that recognizes those who "demonstrate exceptional leadership in promoting the value of older adults with opportunities for education, wellness and spiritual growth". The award was presented at a luncheon February 20, 2008.
- 2007 Winner of Business Journal Mid-Market Growth Award.
- State chooses Community Care to continue the expansion of managed, long-term care services in Five Southeastern counties.
The counties to be served by Community Care include: Sheboygan beginning operations on February 1, 2008, followed by Ozaukee county in March 2008, Washington county in April 2008, and Waukesha county in July 2008. Walworth county operations will begin in 2009. All counties will operate the Family Care program, others will operate the Family Care Partnership Program and/or the Program of All Inclusive Care for the Elderly (PACE). These programs serve elderly, physically disabled and developmentally disabled populations.
- State chooses Community Care as Care Management Organization for Racine and Kenosha Counties.
On January 1, 2007 Family Care services to residents of Racine county began. After one year of operation the wait list in Racine county no longer exists! As of February 1, 2007 Family Care services became available to the residents of Kenosha county. The Family Care Partnership program started serving Kenosha residents in April 2007.
- $100,000 Planning Grant to Expand Long-term Care in Southeast Wisconsin.
It was a big day for Community Care on February 2 as the Governor presented a planning grant check at our Kenosha site. Leading the partnership named, Southeastern Wisconsin Care Management Organization is Community Care Chief Operating Officer, Paul F. Soczynski. He along with representatives of Kenosha, Racine, Washington, Waukesha, Ozaukee counties and 3 private organizations will partner together to expand long-term care services for the elderly, physically and developmentally disabled. "The planning grant will support our mission of Community Care, which is to keep people in their homes while making wise use of the limited funds available" says Paul. Many staff and two of our Racine participants were present at the event.
- National 2005 Marie-Louise Ansak Award
Community Care employee, Lucy B. was recognized as winner of the Marie Louise Ansak award by the National PACE Association at their annual conference in Denver on October 25, 2005. The award is named after Marie Louise Ansak, the “grandmother” of PACE (Program of All Inclusive Care), any employee that provides direct care at any one of the 40 sites nationally is eligible to be nominated. Lucy works as a Rehab Assistant at our Prospect Place group home and was nominated by her supervisor.
- End of Life Award
The Palliative Care Program at Froedtert Memorial Lutheran Hospital and the Medical College of Wisconsin celebrated its 10th anniversary in 2003 by recognizing Community Care with its Community Leadership Award in End of Life Care. Community Care’s End of Life program provides our participants and their families end of life care centered on their needs and wishes and includes services of a Palliative Care Specialist and Chaplain. This program was developed in collaboration with Froedtert Memorial Lutheran Hospital and the Medical College of Wisconsin Palliative Care Program.
- Layton Boulevard Corridor of Aging Excellence
The Milwaukee community was selected by the Robert Wood Johnson Foundation as one of 8 sites nationally to receive funding through the Foundation’s Community Partnerships for Older Adults program. The Milwaukee County Department on Aging was awarded the $750,000 grant for development and operation of the Connecting Caring Communities initiative. Community Care is one of 5 primary partner organizations involved in this initiative and has been charged with addressing the concept of developing an “elder friendly neighborhood” to create a corridor of excellence and has piloted the initiative within the Layton Boulevard neighborhood.
- National Social Advocacy Award
The American Planning Association recognized the Lapham Park Venture, our collaborative project with the City of Milwaukee and other partners, with the 2004 National Social Advocacy Award for the commitment it demonstrates to planning for the needs of less fortunate members of society.
