Community Care programs in Milwaukee and Manitowoc counties

Under contract with the Milwaukee County Department on Aging, Community Care serves as the Care Management Unit of the Milwaukee County Family Care program. Staff includes Registered Nurse Case Managers, Social Work Case Managers and a Unit Manager.

Two additional Community Care programs provide case management and brokered in-home services to Milwaukee County residents who are elderly and disabled, but want to remain in their homes:

  • Grandfathered Community Care Demonstration Program, which was the model for the COP waiver program adopted by the State of Wisconsin
  • Comprehensive Community Care Program, supported by United Way

Community Care also provides case management services to elderly and disabled people in Manitowoc county through the County Department of Human Services.

More than 40 Community Care employees work in these Community Care programs, which serve approximately 1,200 elderly or disabled people.

Community Programs

Comprehensive Community Care Programs

Our Comprehensive Community Care Programs enable elderly and disabled adults to remain independent through case management services and the provision of affordable in-home and community-based services. The target population served is low income and minority adults for whom such care is either unaffordable or not readily available. This program is often used to provide interim care for individuals until they are eligible for publicly funded services. Funding is provided through United Way allocations and client copayments based on a sliding fee schedule.
