Pharmacy Network

Community Care Partnership provides medications through its Medicare Part D Program. In order
to ensure you receive the best possible medication services, Community Care has Pharmacists and
PCPs who work with you to make sure you are on the right medications for your medical
conditions. They also carefully monitor the interaction between all of the medications you are
taking to prevent side effects.

Your Community Care team and the pharmacy will work with you to provide the best
pharmaceutical services possible including periodic reassessments of the medications you are
receiving, monitoring for interactions and developing systems to help with the adherence to your
medication regimen.

You can most easily obtain your medications by calling into your team pharmacy number to obtain
refills or if you have any questions about your medications. You can also speak to your nurse or
Community Care PCP if you have any questions or concerns about the medications you are on.
This is the primary pharmacy for your medications. Community Care will bill you monthly for any
copay you may have.
