E-forms Claims Submission is the option for non-medical providers to electronically submit claims to Community Care. Simply complete the “E-Form” online, “click” submit, and receive a tracking number that is unique to each claim you submit.

Training session to help you understand how to submit your claims via eForms are held monthly. Upcoming session are listed below:
