Provider Bulletin | August 2010

Welcome to the August edition of the Provider Bulletin. In this edition you will find useful information about OSHA inspections and citations, prevocational definition training, and upcoming training classes on submitting claims.


OSHA Inspections and Citations

OSHA has started inspections and we wanted to share with you some of the citations that are being issued. Assisted Living (CBRF, RCAC, licensed AFH) is now a part of the OSHA-focused inspection list with the main focus being ergonomic stressors, exposure to blood and other infectious materials, slips, trips and falls, and exposure to Tuberculosis. Listed below are some reminders based on citations that assisted living facilities are currently receiving.

  • Ensure that all fields on the OSHA 300 are filled in: Case number, Employee name, Job title, Date of injury, where the event occurred, and description of injury/illness.
  • Record number of days away from work, transferred, or restricted.
  • Be specific in the description of where the event occurred, e.g., rather than stating “hallway,” state, “employee entrance at north end of building.”
  • Be specific in the description of the injury. For example, rather than stating “back pain,” state, “employee was assisting to transfer resident in Room 301 to the bedside chair and felt pain in back.”
  • Record all sharps injuries on the OSHA 300 as well as a separate Sharps Injury Log. The Log should include date of incident, type and brand of device being used at the time, department or area where the incident occurred, and a description of how the incident occurred. At least annually, management should be reviewing the type of needle devices that are involved in sharps injuries to determine if a change in device is indicated. Employee input during this process is very important.
  • Don’t record the employee’s name on the OSHA 300 if the injury is of a sensitive nature (e.g., bloodborne exposure). Maintain a separate Privacy Log where the employee’s name can be recorded and crossreferenced with the case number from the OSHA 300.
  • Familiarize yourself with OSHA definitions of first aid cases vs. medical cases to avoid over-recording.
  • Post the Summary Report (OSHA Form 300A) from February 1st to April 30th of the following year.

Ensure that written programs are up-to-date and are specific to your facility (not just boilerplate templates). While not all inclusive, these are programs that should be in writing:

  • Bloodborne pathogens. Be sure to include post-exposure follow-up protocols, as well as the providers who will be doing the post-exposure follow-up. If you are a multi-location company, providers for each site should be listed in the Plan.
  • Hazard Communication.
  • Lockout/tagout. This standard applies any time an employee has to remove a faceplate, guard, or cover and has potential contact with energized parts.
  • Personal protective equipment. Be able to demonstrate that an assessment for appropriate PPE has been done for all departments.

DME/DMS Bench

All DME/DMS providers must submit a current MSRP along with their quote to the IDT team for all miscellaneous items prior to authorization issuance.

Community Care conducts claims education training to providers on a monthly basis. You are welcome to attend any of the sessions listed below. Current and new non-medical providers are welcome.

Evidence of Insurance Declaration

Community Care Network Providers are required to provide Evidence of Insurance Declaration to Community Care at 12-month intervals for all contracted entities. Copies of the following are mandatory:

• General Liability
• Professional or Umbrella Liability
• Worker’s Compensation & Employers Liability Insurance (if three or more employees)
• Vehicle Liability Insurance

Prevocational and Employment Service Providers

The Department of Health Services (DHS) has developed an on-line Prevocational Definition training that provides additional information on the definition, including guidelines for implementation. The training is in six modules, each approximately 10 minutes long. The training is open to anyone and is accessible via this link: http://www.dhs.wisconsin.gov/LTCare/ProgramOps/Index.htm

Training is found under section “Technical Assistance Series 10-04.” The training also allows for comments and questions to be submitted and responses to the questions will be posted on-line. Providers are strongly encouraged to view this training.
