Community Care serves new areas

This year Community Care is rapidly expanding its services, providing managed long-term care to four new counties in 2008: Ozaukee, Sheboygan, Washington and Waukesha counties.

Last fall, the state of Wisconsin awarded Community Care with a $44.5 million contract to develop and operate managed long-term care services in those counties through Family Care and Family Care Partnership, programs which serve adults with physical or developmental disabilities and frail older adults.

Recent program launches in 2008 include:

  • Sheboygan County in February;
  • Ozaukee County in March;
  • Washington County in April;
  • Waukesha County in July.

Community Care is the only managed care organization in Wisconsin to provide PACE, Family Care Partnership and Family Care Programs.
