FPCMC honors Community Care’s Paul Soczynski’s leadership

The Family Partnership Care Management Coalition (FPCMC) celebrated its last planning session for long-term care reform on Aug. 20. At that meeting, the FPCMC honored the leadership of Paul Soczynski of Community Care.

The celebration was held at the Dodge County Administration Building in Juneau, WI. The group will now transform to an advisory committee for the ongoing management of Family Care and Family Care Partnership in the 12 counties it serves: Columbia, Dodge, Green Lake, Jefferson, Marquette, Ozaukee, Sauk, Sheboygan, Walworth, Washington, Waukesha and Waushara.

FPCMC was organized in mid-2005 to begin planning for long-term care redesign. The coalition grew out of the Family Partnership Initiative, with 15 counties and Lutheran Social Services joining forces.

In planning for long-term care reform, they were building on the experience of existing, local county long-term care staff and programs, in coordination with community nonprofit partners who have proven experience in high quality, integrated managed care.
