Our News 1,000 and Counting Community Care reached a major milestone this month. Our 1,000th employee was hired. In the past two years, Community Care has expanded into 11 counties, which is significant since our primary area of focus just a few years ago was only Milwaukee and Racine counties. In our 11 counties, we serve the long-term care and health needs of over 9,000 members in the Family Care, Partnership, and PACE programs. Community Care has long been a progressive employer, offering compensation, benefits and an internal promotion policy designed to attract and retain hard-working, talented and caring employees. According to the mission statement of our Human Resources Department, Community Care provides “a work environment characterized by fair treatment of staff, open communications, personal accountability, trust and mutual respect… We are committed to excellence in service, and strive to continue to develop our diverse workplace.” Alicia Modjeska joins Community Care, Inc. as Chief Operations Officer Community Care is pleased to announce the appointment of Alicia Modjeska to the position of chief operations officer. Modjeska comes to Community Care from Westside Healthcare Association where she served as the executive vice president and chief operations officer. Modjeska is also a former administrator of St. Michael’s Hospital. Modjeska has a BS in Nursing and a Master’s Degree in Health Services Administration. She has an impressive track record of outstanding accomplishments and we look forward to her proven leadership and the many contributions she will make to Community Care and the more than 8,000 members we serve. Groundbreaking Begins Construction of Our Lady of the Angels
New Adult Day Health Center opens in Waukesha Community Care announces the opening of our new adult day health center located at 1801 Dolphin Drive in Waukesha. The new center will serve Waukesha residents in both our PACE and Partnership programs beginning March 1, 2009. The new center includes a rehab gym, special dementia area, a clinic with 4 exams rooms, speciality exam room, a lab and med room. Day center participants will enjoy socializing, therapeutic recreation, a hot lunch and the chance to sit on a beautiful patio overlooking a natural wetlands. Partnership program comes to Ozaukee and Washington Counties The disabled and elderly residents of both Ozaukee and Washington counties now have another choice to fullfill their healthcare needs. The Family Care Partnership program started accepting members in January of 2009. The program provides all inclusive healthcare and services. A clinic is located in our West Bend location. The Coalition of Wisconsin Aging Groups (CWAG) Awards Kirby Shoaf and Community Care. On November 6, Community Care received an award at the The Coalition of Wisconsin Aging Groups (CWAG) 2008 Governing Board banquet and award ceremony. The President's Award, presented by Karen Robison, President of the Governing Board and Tom Frazier, Executive Director of CWAG was awarded to our CEO, Kirby Shoaf for his 30 plus years of longterm care contributions to the residents of Wisconsin. State chooses Community Care to continue the expansion of managed, long-term care services in Five Southeastern counties. The counties to be served by Community Care include: Sheboygan beginning operations on February 1, 2008, followed by Ozaukee county in March 2008, Washington county in April 2008, and Waukesha county in July 2008. Walworth county operations will begin in 2009. All counties will operate the Family Care program, others will operate the Family Care Partnership Program and/or the Program of All Inclusive Care for the Elderly (PACE). These programs serve elderly, physically disabled and developmentally disabled populations. State chooses Community Care as Care Management Organization for Racine and Kenosha Counties. On January 1, 2007 Family Care services to residents of Racine county began. After one year of operation the wait list in Racine county no longer exists! As of February 1, 2007 Family Care services became available to the residents of Kenosha county. The Family Care Partnership program started serving Kenosha residents in April 2007. Community Care purchases new site at 3220 W.Vliet Street. After searching for a very long time a building that was formerly the home to the Industries for the Blind was purchased by Community Care on June 29, 2006. The building had been used for light manufacturing and offices. The 95,000 square foot building will be renovated to allow for many uses including: an adult day health center, a clinic, general office space, a kitchen, central storage and employee fitness center. Torke, Wirth and Pujara is the architect working on the project. JENS Construction is the General Contractor. Opening is scheduled for May 2007.
