Groundbreaking Begins Construction of Our Lady of the Angels

Construction has begun on Our Lady of the Angels Memory Care Facility located at 92nd and Howard in Greenfield. This 48 bed facility is owned by the Sister communities we work with in partnership with Clement Manor. All residents of this memory care facility will be enrolled into the PACE program and Community Care will provide all of the direct care staff as well as the PACE care management and primary care responsibilities. Construction will take place over a period of six to nine months with an opening targeted in early 2011.

New Adult Day Health Center opens in Waukesha

Community Care announces the opening of our new adult day health center located at 1801 Dolphin Drive in Waukesha. The new center will serve Waukesha residents in both our PACE and Partnership programs beginning March 1, 2009. The new center includes a rehab gym, special dementia area, a clinic with 4 exams rooms, speciality exam room, a lab and med room. Day center participants will enjoy socializing, therapeutic recreation, a hot lunch and the chance to sit on a beautiful patio overlooking a natural wetlands.

Partnership program comes to Ozaukee and Washington Counties

The disabled and elderly residents of both Ozaukee and Washington counties now have another choice to fullfill their healthcare needs. The Family Care Partnership program started accepting members in January of 2009. The program provides all inclusive healthcare and services. A clinic is located in our West Bend location.

The Coalition of Wisconsin Aging Groups (CWAG) Awards Kirby Shoaf and Community Care.

On November 6, Community Care received two awards at the The Coalition of Wisconsin Aging Groups (CWAG) 2008 Governing Board banquet and award ceremony. The President's Award, presented by Karen Robison, President of the Governing Board and Tom Frazier, Executive Director of CWAG was awarded to our CEO, Kirby Shoaf for his 30 plus years of longterm care contributions to the residents of Wisconsin. The second award, Community Partner of the Year was accepted by Chief Operations Officer, Paul Soczynski on behalf of Community Care.

State chooses Community Care to continue the expansion of managed, long-term care services in Five Southeastern counties.

The counties to be served by Community Care include: Sheboygan beginning operations on February 1, 2008, followed by Ozaukee county in March 2008, Washington county in April 2008, and Waukesha county in July 2008.  Walworth county operations will begin in 2009.   All counties will operate the Family Care program, others will operate the Family Care Partnership Program and/or the Program of All Inclusive Care for the Elderly (PACE).  These programs serve elderly, physically disabled and developmentally disabled populations.

State chooses Community Care as Care Management Organization for Racine and Kenosha Counties. 

On January 1, 2007 Family Care services to residents of Racine county began.  After one year of operation the wait list in Racine county no longer exists!  As of February 1, 2007 Family Care services became available to the residents of Kenosha county. The Family Care Partnership program started serving Kenosha residents in April 2007.

Community Care purchases new site at 3220 W.Vliet Street.

After searching for a very long time a building that was formerly the home to the Industries for the Blind was purchased by Community Care on June 29, 2006.  The building had been used for light manufacturing and offices.  The 95,000 square foot building will be renovated to allow for many uses including: an adult day health center, a clinic, general office space, a kitchen, central storage and employee fitness center.  Torke, Wirth and Pujara is the architect working on the project. JENS Construction is the General Contractor. Opening is scheduled for May 2007.

$100,000 Planning Grant to Expand Long-term Care in Southeast Wisconsin.

It was a big day for Community Care on February 2, 2006, as the Governor presented a planning grant check at our Kenosha site.  Leading the partnership named, Southeastern Wisconsin Care Managment Organization is Community Care Chief Operating Officer, Paul F. Soczynski.  He along with representatives of Kenosha, Racine, Washington, Waukesha, Ozaukee counties and 3 private organizations will partner together to expand long-term care services for the elderly, physically and developmentally disabled.  "The planning grant will support our mission of Community Care, which is to keep people in their homes while making wise use of the limited funds available" says Paul.  Many staff and two of our Racine participants were present at the event.
