For Community Care 2009 was a year of great successes - our members' successes!

2009 Milestones

We began providing Family Care services to residents of Milwaukee and Walworth Counties. We also introduced the Partnership program to residents in Ozaukee and Washington Counties.

In Waukesha County, residents were offered two new options, Partnership and PACE (Program of All Inclusive Care for the Elderly).

Community Care surpassed 800 total employees serving the health and well-being needs of over 5,400 members.
